Our Online Pre Appointment Questionnaires:
If you are a new client to Tracy James Hearing then our online questionnaire’s MUST be completed prior to each Wax removal and Hearing appointment. The link to the questionnaire form will be in your confirmation email. If you do not receive a confirmation email, check your spam folder and then contact us on admin@tjaudiology.com so we can send you the appointment information and questionnaire link directly.
Please Note: If you book your appointment whilst you are visiting another country the time difference will affect the booking time.
